Medical Insurance Submissions
It is the client’s responsibility to contact the insurance company to
find out their insurance benefits. The number to call is on the back of your insurance card.
Prior to the
first session you may want to check if you have to pre-certify and ask your representative details about your annual
deductible, the % co-pay and how many sessions you are allowed per year.
An insurance company may not pay for services
that they consider to be non-efficacious, not medically or therapeutically necessary, or ineligible (not covered by your policy,
or the policy has expired or is not in effect for you or other people receiving services).
Payment is due at the time of service. Dr. Allen will then either submit
the details to your insurance who will reimburse you directly or provide you with a statement that contains all the relevant
information for you to submit to your insurance company yourself for reimbursement. Please complete the insurance submission form below if you would like me to submit your claim on your behalf.
